Academic Internship Evaluation
Please complete an evaluation for each student performing an internship at your company. Evaluations are requested by University policy and are used to help assign a grade for the student who is earning credit for internship work. Submitted forms are confidential, sent directly to the faculty internship coordinator for Chemistry and Biochemistry, and not shared with the student.
